You will need to apply in person, but your application can be filled out on-line and printed before you go if you would prefer to do it that way.
Fees are as follows:
Certified Birth Certificate: $26.00 (get this at the treasurer's office in the courthouse)
Murray County Court Administrator: $25.00
US Department of State: $110.00
Once you receive your passport and birth certificate back bring both documents, along with your driver's license if you have one, to Mrs Johnson for photocopying. This is done so that group leaders have the necessary documents in case they are needed during the trip.
